Created Unassigned: Cannot Add Site Collection [25986]

I am attempting to connect to SPOnline using standard login (ie. user@mytennant.onmicrosoft.com) but the following error shows up. Earlier versions of the app usually worked not problem. I am using 1.5.1.0
